  Patient Types:
  Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes, and perhaps a hospice patient in an AAH Hospital.
  Hospice patients/end of life (life expectancy 6 months or less if disease runs its normal course).
