Epic Travel Staffing is hiring a Travel - PT Home Health

Shift: -Variable Days / 8x5, 40hrs per week
Length: 13 weeks

Guaranteed Hours per Week: 38 with up to 40 as needed
No weekends or holidays needed -
Requirements:

• WI License1 year experienceHome Health experience
• Training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis
• Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical TherapyMaster's Degree in Physical TherapyAHA BLSOther details:
  • Float required
  • Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
  • Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. - Population served includes adult and geriatric patients. - Documents to meet regulatory and reimbursement requirements. - Meets productivity expectations of an average of 4.5 visits per day
